• Official Receipt

STEPS

1. An individual who needs Health Certificates must complete the required laboratory tests and present the results to the MHO-Sanitation Section
2. Evaluation of the laboratory results
3. Proceed to the Municipal Treasurer’s office for corresponding fee
4. Issuance of the Health Certificate
