STEPS
- Request for PSA form of document/s to be requested from the MCR Office
- Fill up the PSA form with the correct details and submit once finish
- Pay the service fee in the Treasurer’s Office
- Submit the Receipt to the MCR Office
- Submit the Acknowledgment Slip a week after the date of request
