REQUIREMENTS

• Negative Result from PSA
• Baptismal Certificate (2 photocopies)
• Voter’s Registration Record
• Affidavit for Delayed Registration
• Affidavit of Two (2) Disinterested Persons
• School Records (TOR, Form 137)
• Duly Accomplished COLB
• Community Tax Certificate (CEDULA) or any government-issued ID of both parents

STEPS

1. Submit the duly accomplished COLB in the Office of the MCR
2. The client should sign the Affidavit for Delayed Registration and both parents will sign the Affidavit of Acknowledgement / Admission of Paternity at the back of the COLB
3. Proceed to the Treasurer’s Office for corresponding fee
4. Submit the Receipt to MCR Office
5. Submit the claim stub and claim the registered COLB in the MCR’s Office
