TIMELY REGISTRATION OF CERTIFICATE OF LIVE BIRTH (COLB) if parents were NOT MARRIED but child using the father’s last name
REQUIREMENTS
- Duly accomplished COLB
- Affidavit to Use the Surname of the Father (AUSF) executed by the child’s mothe
- Community Tax Certificate (CEDULA) or any government-issued ID of both parents
STEPS
- Submit the duly accomplished COLB in the Office of the MCR
- The mother of the child will execute the AUSF
- Submit the duly accomplished COLB and AUSF with signatures
- Procced to the Treasurer’s Office for the corresponding fee
- Submit the Receipt to the MCR Office
- Claim the registered COLB
