• NBI Clearance or Police Clearance
• Community Tax Certificate
• Medical Examination Result
• 1×1 Picture (2 pieces)

STEPS

1. Submit all the requirements to the BPLO Personnel
2. Proceed to the BPLO and fill out forms for processing
3. Proceed to the Treasure’s Office for the corresponding fee
4. Await for the release of Occupational/ Mayor’s Permit
