REQUIREMENTS

• MERALCO Inspection Form (Yellow Card)
• Duly accomplished Certificate of Completion (Electrical Works) Form
• 2 Photocopies of the approved Electrical Permit
• Photocopy of PRC ID and PTR of Duly Registered Professionals who signed the plans and documents, with original signed and sealed

STEPS

1. Submit the complete requirements
2. Accompany the Electrical Inspector in the inspection of the site/ project
3. Proceed to the Cashier at Office of the Municipal Treasurer for corresponding fee
4. Present the Official Receipt (OR) to the Building Official of the Municipal Engineering Office
