REQUIREMENTS

• MERALCO Inspection Form (Yellow Card)
• Duly accomplished Certificate of Completion (Electrical Works) Form
• Duly accomplished Electrical Wiring Permit (if the application is for remodel, up-grading reconnection, connection, relocation, net metering, burn-out meter, separation of service entrance, addition of meter)
• Barangay Clearance for Wiring Permit
• Homeowners Association Clearance, if any
• Photocopy of Certificate of Occupancy
• Photocopy of Transfer Certificate of Title
• Photocopy of the latest Tax Declaration and  Real Property Tax Receipt
• As-Built Electrical Plans (signed and sealed)
• Photocopy of PRC ID and PTR of Duly Registered Professionals who signed the plans and documents, with original signed and sealed
• Note:
  1. Lot Owners Consent (if lot is not owned by the applicant)
  2. Undertaking from the Municipal Mayor (if the lot is a government property)
  3. USDO Certification (If Informal settlers)

STEPS

1. Submit the complete requirements and accomplish the Wiring Permit Form
2. Accompany the Electrical Inspector in the inspection of the site/project
3. Proceed to the Cashier at Office of the Municipal Treasurer for corresponding fee
4. Present the Official Receipt to the Building Official of the Municipal Engineering Office
