REQUIREMENTS

• Duly accomplished and Notarized Certificate of Completion Form
• Fire Safety Inspection Certificate
• Photocopy of all permits applied
• Photocopy of all Zoning, Fire Clearance & Official Receipts
• Plans and Documents signed and sealed over the printed name of the duly licensed and registered professionals.
  1. As Built-Plan – One (1) set if no changes, three (3) sets if with changes 
  2. Construction Logbook
  3. Pictures of the Structure (Front, Rear, Left and Right Side)
• 3 Photocopies of PRC ID and PTR of Duly Registered Professionals who signed the plans and documents, with original signed and sealed
• LLDA Discharge Permit (For Commercial and Industrial)

STEPS

1. Submit all necessary requirements
2. Accompany the Engineering Staff for the inspection of the constructed building structure. If the structure complies with the approved plans the owner receives the order of payment
3. Proceed to the Cashier at Office of the Municipal Treasurer for payment
4. Present Official Receipt to the Building Official
