• Certificate of Solo Parent
• Photocopy of Birth Certificate
• Photocopy of Death Certificate (if spouse is deceasd)
• 1×1 and 2×2 ID picture
• Affidavit of Solo Parent
• Filled-out application form

STEPS

1. Attend Orientation on Expanded Solo Parent Welfare Act or RA 11861
2. Comply and submit complete requirements base on solo parent Category.
3. Checking and assessment of submitted documents
4. If needed, conduct home visitation to verify the eligibility as a solo parent
5. Fill up Solo Parent application Form
6. Approval of Solo Parent application fom
7. Issuance of Solo Parent ID’s
