• PWD’sID and claimant ID’s
• Barangay Indigency Certificate indicated the purpose
• For Laboratory Examination, Laboratory Request
• For Hospitalization, Billing Statement and Medical Abstract
• For Burial Assistance, Registered Death Certificate
• For Person with Autism/ Person with Intellectual Disability
  • – Request for Psychological Evaluation

STEPS

1. The client / claimant submit the complete requirements at the MSWDO.
2. Interview and initial assessment
3. Fill up of AICS form (If the requirement is complete)
4. Checking and processing the application
5. Approval of the application
6. Waiting of schedule for release of financial/ medical assistance
